If the available times below do not work, please call our office at 716-688-0020. Our staff will do their best to accommodate your needs.
While Scheduling:
- Name: Enter your name exactly as it appears on your health insurance card.
- Reason for Visit: Provide a brief and clear description of the issue. Please note: only one concern can be addressed per visit.

IMPORTANT Information for scheduling:
Visit Type
- Cosmetic Services: For patients looking to have a Cosmetic Consultation, Dysport/Botox, and/or Fillers.
- Established Patient – Medical: A patient who has been seen in our office within the last 3 years.
- New Patient – Medical: A patient who has never been seen in our office or has not visited in over 3 years.
Visit Reason
- Follow-Up: For established patients receiving continued care for a previously treated condition.
- Skin Check – Full Body: A comprehensive, head-to-toe skin evaluation. Recommended to have at least once a year.
***Please remember to bring your photo identification and health insurance card to the appointment. All minors must be accompanied by a parent or legal guardian. All copays are due at the time of check out. All appointments require 24 hour notice to cancel or reschedule an upcoming appointment, if cancelled or rescheduled less than 24 hours a $50 fee will be assessed to patient.***
