Important Scheduling Instructions:
If the available times below do not work, please call our office at 716-688-0020. Our staff will do their best to accommodate your needs.
While Scheduling:
- Name: Enter your name exactly as it appears on your health insurance card.
- Reason for Visit: Provide a brief and clear description of the issue. Please note: Only one (1) concern can be addressed per visit.

IMPORTANT Information for scheduling:
Visit Type
- Cosmetic Services: For patients looking to have a Cosmetic Consultation, Dysport/Botox, and/or Fillers.
- Established Patient – Medical: A patient who has been seen in our office within the last three (3) years.
- New Patient – Medical: A patient who has never been seen in our office or has not visited in over three (3) years.
Visit Reason
- Follow-Up: For established patients receiving continued care for a previously treated condition.
- Skin Check – Full Body: A comprehensive, head-to-toe skin evaluation. Recommended to have at least once a year.
Please Note:
- Please bring your photo ID and insurance card to your appointment.
- Patients under 18 must be accompanied by a parent or legal guardian.
- Copays are due at the time of your visit.
- Please provide at least 24 hours notice if you need to cancel or reschedule your appointment. Appointments cancelled or rescheduled with less than 24 hours notice will incur a $50 fee.
