We welcome you as a new patient, or returning patient, to our practice! This page is filled with forms regarding your upcoming visit to our practice. These forms will help in expediting the check-in process.
For new patients, or those that have not updated paperwork in over a year, please log in to the patient portal or print and fill out the following forms before your appointment:

- Notice of Privacy Practices
- Authorization for Release of Protected Health Information (PHI) to Lisa S. Ball FNP PLLC
- Authorization for Release of Protected Health Information (PHI) from Lisa S. Ball FNP PLLC
